Tax Forms: What You Need to Know

Official Notice Regarding 1095-C Health Insurance Forms

The Phoenix Central School District is providing the following update to all staff members regarding the distribution of 1095-C forms for the current tax year. While these forms serve as important documentation of a health insurance offer, please be advised that employees are no longer required to have a physical copy of the 1095-C in their possession in order to file annual tax returns.

Staff members who wish to receive a hard copy of their form for personal record-keeping may do so by submitting a formal request through the 1095-C Request Form.
